The extensive cuts and underfunding found throughout the NHS have drastically affected the remote Scottish island of Barra. The only hospital on the island is St Brendan’s, located in Castlebay, which was declared “completely unacceptable” by NHS Western Isles in 2017 (“New £18.2 million…”, The Press and Journal, 2017). The facility hosts no specialised consultants or equipment, not even an X-ray machine, and is ran by a small rotating team of GP’s. The lack of specialist care means pregnant woman or cancer patients, for example, must travel up to Stornoway or Glasgow to receive treatments. The hospital essentially functions as a GP practise rather than a hospital, unable to provide any serious medical care, instead deferring patients to other facilities.

The facility is shared with a local council care home, which is also far below standard. In August 2025 a dementia patient was able to escape the facility undetected, due to insufficient security measures, and later died following injuries sustained from several falls (“Council fined after death of care home resident, BBC News, 2025).

New plans to replace the existing facilities have been considered since 2017. Financial troubles have plagued the project and as of 2025 the plans are permanently on hold with locals still desperate for new healthcare facilities (“Islanders call for…”, BBC News, 2025).

Staff retention is a serious problem, with NHS Western Isles struggling to keep trained doctors on location for long. Hospitals on the mainland have access to more modern facilities, larger teams and opportunities to do further training. Remote hospitals typically have outdated or lacking equipment and the staff base is often very small and overstretched. Larger cities are more sociable, especially for younger doctors who may not be drawn towards the slow rural lifestyle. The current state of remote NHS hospitals offers little incentive for doctors to relocate to the isles, far away from friends and family.

Access to other health services is incredibly limited, the nearest hospital providing speciality care is in Stornoway, several hours by car and often restricted in poor weather. In emergency situations patients often must wait hours until an air ambulance can arrive, in the meantime being kept alive with incredibly limited resources in the outdated St Brendan’s.

In 2023 a tractor tire bursting resulted in a Barra resident being described as looking like “a bomb victim”. After being ambulanced to St Brendan’s, the nearest emergency team was still over 200 miles away in Glasgow. The man was kept alive, with limited equipment, for five hours before the team could arrive to take him to Glasgow (“The Scottish islands desperately seeking doctors”, BBC News, 2023).

Events like this are often more common in Barra due to most residents working on the land or sea. Overall Barra is crying out for a more centralised local hospital that can better meet the needs of the island.
